Unless we will have online forms ready within a few days,
I would say we go for it.

P.S.: I would also suggest that we create the file in HTML format
instead of PDF format due to the following reasons:

-If I want give a specific internal location of the file to
  someone (for example, pointing someone on ebXML work in
  e-Government area), giving it in URL form is much easier
  than "explaining to them how to find the same information
  in the PDF file"
-We could divide the document into several other HTML files
  each of which can be seperatedly managed and created
-HTML document in general is more easier to navigate
