Message Header & Envelope

I just joined this forum and read the Message Envelope and Message
Header Specifications.

I hope that someone can help me understand the following:

In the Envelope Specifiaction there is an example that has a ebxmlheader
container and ebxmlpayload container that also has a header section.
What is the relationship between the headers.
I also did not find in the Header Specifiaction  coverage for elemnts
that were in the payload header such as ReplyToUri and Token.

What did I miss ?
