Comments on ebXML Messaging Services spec v0.21d

While debugging the jEBX Message parser I think I came across a few
minor errors in the dtd for both the header and error specifications. 

Here are my notes:

About the jEBx parser - besides handling the ebXMLHeader object it now
handles ebXMLError objects.  The package was updated on my website this
morning.
