Other specs have the same issue.  It would be nice if some folks who are
active in multiple groups would suggest a standard. :)

My real question is related to David's.  For those elements that need an
ID, should we use the XML "id" attribute, or define a new id attribute. 
The bits and pieces of the CPP/CPA, for example, seem to create fooid
(and sometimes fooref) attributes for foo elements.  I'd like to see
that style question addressed, if possible.
