RM section redraft

All,

Here's my redraft of the RM section. I am aware that it doesn't
include the sequence number considerations. These should be 
added in in Vancouver during our f2f.

Also, we need to make it clearer regarding the "black-box" nature
of what happens when multiple MSH instances are involved in the delivery
of a message.
