Skeleton Message Header Specification

Folks

I attach a very high level skeleton for the structure of our first
deliverable. I've used the same type of headings that are used by IETF
Internet Drafts. I've also separated out the Message Envelope from the
Message Header specification since:
a) there has been some debate on what the envelope should look like
b) we might have many different types of envelope
c) if we specify it in a separate document we can more easily develop it in
parallel and revise it to take account of new developments (e.g. the W3C)

Finally I've sent it out in MS Word format since I reckon that people will
want to revise it - if this causes anyone a problem let me know and I'll
issue a PDF version.
