Lines 637-646, especially 644-646.

Whereas the TraceHeaderList element is 'appended' to the TraceHeader at each
MSH hop, the 'TraceHeader' must be read and rewritten at each MSH hop.  To
accomplish this, the SOAP actor global atribute (set to the special URI
"http://schemas.xmlsoap.org/soap/actor/next" ) is a required attribute on
"TraceHeader".  Lines 644-646 do not list this as a required attribute.
